Timesheet and Expenses Claim Form

By submitting these expenses I confirm I have incurred the time sheet expenses wholly, necessarily and exclusively for performing my duties of employment.
I also confirm (where applicable) that a cost in respect of a meal (food and drink) was incurred after starting the business journey on each day the benchmark scale rate for subsistence is claimed.
